- Authorized and issued shares (if corporation)
- Paid-in capital
- State (or country) of Incorporation
- Period of duration
- Date of formation in state of incorporation
- Street address of principal office, wherever located
- Street address of principal office in Illinois
- A designated commercial registered agent for service of process (can be either a corporation or natural person)
- Street address of agent for service of process (PO Box is NOT acceptable)
- States and countries in which the company is admitted or qualified to transact business (include state of incorporation)
- Nature of business / type of services or products offered
- Signature and title of the authorized person

- Certified copy of Articles of Incorporation (within the last 90 days)
