How do I complete the USPS 1583 form?

ACTION REQUIRED ON YOUR PART

Before we can issue your Delaware address and Private Mailbox Number (PMB#), the United States Postal Service requires that you complete the "Application for Delivery of Mail Through Agent" form (USPS form#1583).

STEPS TO COMPLETE:

Please complete USPS form #1583. Your signature MUST be NOTARIZED by a Notary Public (A Notary Public from any US state is acceptable.  If you live outside the US, any official authorized to witness oaths is acceptable.) Please return the duplicates bearing your notarized original signature to us by scan/email, fax or mail Two (2) forms of acceptable identification (see below); one of the identification documents must contain a photograph
