STEP 2: Once you've filled in the PS 1583 form, either SAVE the document to your desktop or Print to PDF.
STEP 3: Your completed PS 1583 form must be notarized. We suggest using our electronic notarization vendor Notarycam. The process is easy and fast. Notarycam charges $25 for each electronic notarization (each document) if you are located within the United States. They charge $79 per electronic notarization (each document) if you are located outside of the United States during meeting. They accept all major credit cards. STEP 4: Go to www.NotaryCam.com/dbiglobal (click here) website and complete a few fields of information. Upload your completed PS 1583 form to Notarycam.
STEP 5: Notarycam will contact you to make an appointment for a webcam session. You must have a computer with camera or you can use your smartphone with camera.
STEP 6: Be sure to have two acceptable IDs:

- Driver’s license or state non-driver’s identification card;
- Armed Forces, government, university or recognized corporate identification card;
- Passport; alien registration card or certificate of naturalization;
- Current lease, mortgage or Deed of Trust;
- Voter or vehicle registration card;
- Home or vehicle insurance policy.
Important: At least one of the IDs must have a recent photo. Please be sure IDs are COLOR copies.
STEP 7: After your webcam meeting, Notarycam will email the notarized PS 1583 form to us with your two forms of identification.
STEP 8: Upon receipt of the email from Notarycam, we will issue a Suite Number or PMB Number. Do NOT use the address until you have received your Suite Number or PMB Number.
STEP 9: Postage and scanning charges are not invoiced. You will receive a Welcome Email so that you can log in to our Customer Portal (click here) where you can provide a credit card or e-Check (ACH) that we can use to charge your postage and scanning fees.
