We will also need a PS1583 form completed and notarized. It is best if you fill in the Lease Agreement and the PS1583 form and UPLOAD them both to https://www.NotaryCam.com/dbiglobal.com

Please be sure two IDs are COLOR copies.

You'll need to sign the Lease Agreement and PS1582 in front of the e-notary. Notarycam.com will e-notarize the lease agreement and forward the signed and e-notarized document together with your IDs. After we receive the "signed and notarized" lease agreement, the Landlord will sign it and email a copy of the fully executed lease agreement to you. A hard copy will be mailed to you as well. PLEASE NOTE: Once the lease agreement is signed and notarized by you, we will issue the Suite Number to you.
