DBI charges an annual fee of $197 for the Phone Number Service with Utility Bill Proof. Our phone company service provider charges a monthly line charge of $75.00 plus government taxes, fees and surcharges. There is NO installation fee. There is a minimum contract period of 24 months. I understand I will receive a phone number utility bill from the phone company for these services in my COMPANY NAME and at my billing address. Please allow 30-45 business days for setup of the phone number. I understand that I should receive the phone number utility bill within 30-45 days.
